Family Transition Process

The goal of Little Red's transition process is to ensure that there is no gap in services for eligible families after the child's 3rd birthday.

* Enrolled families who have children who meet developmental milestones prior to their 3rd birthday are not eligible for school district services. For these families, Little Red staff will assist them in transition to educational opportunities in the community.

STEP 1: Transition Meeting

Three to six months before the child's 3rd birthday, the Family Resource Coordinator meets with family members to explain the transition process. If available, a representative from the school district will attend. With the family's permission, a transition evaluation will be scheduled.

STEP 2: Transition Evaluation

Within three months before the child's 3rd birthday, the Family Resource Coordinator will use an evaluation tool to review the child's ability in all five developmental areas:

  1. Motor skills
  2. Cognitive skills
  3. Communication skills
  4. Social/Emotional skills
  5. Adaptive skills

STEP 3: School District Evaluation

With the family's permission, the evaluation scores and report will be shared with the school district's Childfind team. This team reviews the evaluation scores and report to determine the child's eligibility for services available through the school district.

Eligible

If the child's evaluation rates his or her abilities as meeting the school district criteria , the school district schedules the Individualized Education Plan (IEP) meeting.

Not Eligible

If the child's evaluation score rates his or her abilities within the range of typically developing children, the child is not eligible for special education services through the school district. At the family's request, the Family Resource Coordinator will work with the family to find other community resources (*link to parent resources page).

STEP 4: individualized Education Plan Meeting

Within three months before the child's 3rd birthday, an Individualized Education Plan (IEP) meeting is held with eligible families and the school district. With the IEP in place, the family can begin school district services immediately after the child's 3rd birthday, so that there is no gap in service.
